Exit Survey
You are invited to participate in our survey on Patient Assistance Programs, conducted by several nonprofit patient organizations working collaboratively, including DPAC, Diabetes Sisters, Children with Diabetes, College Diabetes Network, JDRF, the Endocrine Society, and Beyond Type 1.  Approximately 1,000 people will be asked to complete a survey that asks questions about their knowledge of Patient Assistance Programs and any experiences they might have had. It takes less than 4 minutes to complete.

The results of the survey will be included in a white paper to be published publicly. The intent is for pharmaceutical manufacturers, insurers, and foundations that provide these types of programs to better understand the barriers and outcomes of these programs. We hope this survey will help to improve existing Patient Assistance Programs.

Your participation in this survey is completely voluntary. There are no foreseeable risks associated with this survey. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
